Gas Fire condemned due to unusual hole in the flue

Hello MSE folks,
I was hoping for some help/advice.
I had British Gas out today to service my boiler and do a general safety test on my gas appliances.
I live in a 3-bed semi which was built circa 1970 I have, what I believe is called a pre-cast flue.
The engineer drew my attention to some gaffer tape over one of the flue's concrete blocks in my loft, when we removed the gaffer tape we saw that there was a hole cut out but with the cut out bit lodged in. Stranger still, this hole looks as if it was manufactured into the cement block has it cannot be pushed through and, well, it's too perfect to have been done with a tool. This area has kind of a shelf in it and we assumed the hole was there to retrieve anything that may have fallen down.
The Engineer as a result said there was possibility of leakage from this hole and has now condemned the fire which I totally understand. He also said that in his 20 years of experience he'd never seen it before.
Can anyone shed any light on this?
