Equifax & Clear Score - default?

Hi all
I mainly check my credit file on MSE credit club which is through experian - just to check nothing I am unaware of ever appears.
I recently did an eligibility checker for a 0% balance transfer credit card and it said 70% accepted, so applied and it came back no.
I then checked through clear score to see if Equifax was showing anything and in the 'warnings' bit it said - try to avoid getting any more defaults on your accounts
However when checking through the open accounts it didn't show any? The only thing I can see is 2 x late payments recorded on my HSBC credit card account. (this was agreed with the bank at the time who froze my payments for me for 2 months as I changed jobs due to redundancy) This was in Sep & Oct and I then repaid it in nov bought it up to date and not missed a payment since.
It was my understanding that you had to miss 3 payments to get a default, surely this wouldn't warrant a default? Unsure as to why it is warning me about a default if i can't see one. I tried to log into equifax direct but haven't used it for years and now my account is locked.
Thanks in advance
