Balance transfer from MBNA credit card has failed twice with 2 different companies - why?

So I recently booked holiday flights using my MBNA credit card, knowing I would transfer the balance straight away to an interest free balance transfer card.
I applied for a Sainsburys card first, and the balance transfer request was accepted. For a few days the balance was showing on both the MBNA and Sainsburys cards, then the Sainsburys balance returned to £0. Sainsburys said the payment has been returned to them by MBNA. MBNA said they had not received the payment, had no record of any payment being attempted, and there was no reason for them to refuse the payment. I went back to Sainsburys, we double checked the card details, and tried again. The same thing happened. MBNA assured me there was no issue at their end, so I applied for another balance transfer credit card, this time with Santander. A few days ago I noticed that the balance was showimg on my Santander card, but also on the MBNA card. I contacted MBNA first, who said they have not received any payment request, and again told me they would have no reason to block the payment. Phoned Santander, where we checked the card details are correct (they are). They then carried out a payment trace as they said they have made the payment and as far as they are concerned it has been approved. They couldnt trace the payment so I now have 2 balances on 2 credit cards.
Does anyone have any idea what could be going on here. I'm about to hit the time where I'll have to start paying interest on the MBNA card, and through no fault of my own this transfer just isnt working. I'm guessing the problem must be with MBNA since two separate companies have been unable to make the transfer. But both companies are saying they have never come across anything like this and have no idea what the problem could be.

It's usually the fault of Sainsbury's because they're fairly rubbish with BTs, but it's odd that Santander are also having trouble.
Escalate the issue of the missing payment with MBNA.0
