I got ESA when my sick pay ran out. I sent in the sick note as requested but I never did get that back page signed by anyone. I just ignored it. When my sick note ran out they wrote to me asking for another. No mention was made of that other bit on the form not being signed. They seemed to accept the sick note instead.

Did you fill out the detailed pages of the Work Capability form? Have you been put in the Support Group?
The point of completing the back page s that it is then not necessary to complete the other questions about how your health affects you (if cancer is the only health condition),
