I have re-assessment form to complete (ESA), as my condition has become worse. At the last re-assessment I sent all my evidence. This time shall I just send the new letters received from doctors since last re-assessment or the complete evidence again. Secondly, should just copy word for word of last ESA form and adding extra bits?
Thanks

Why would you copy word for word when you state your condition has got worse?1

I have many illnesses, some require different medicines. I didn't mean exactly word for word the complete form. Changes in medicines, scans, etc I could add.
Plus the evidence from last reassessment or the whole lot?

Are you reacting to a regular demand for re-assessment by DWP or , because your conditions have got worse, have you asked DWP to reassess you ?
You should send everything you have , especially all doctors' ( especially consultants') reports and any other health professionals' comments. In fact actually ask your consultant(s) to write a special report specifically for this new reassessment-------I know you don't have much time but it can be done by talking to the doctor's secretary and asking for what you need as a matter of urgency, especially if you pay for private consultation.
It sounds fine to copy any wording that remains the same as in your last reassessment ( so long as nothing at all has changed). Where anything has changed, you should go into a lot more detail, as it will be new to DWP.
Be prepared for a face-to-face assessment by DWP's medical agents, like Atos etc.
Good luck.0
