You need to flip paragraphs 6 & 7.  There is a  lot more ammunition for the abuse of process point but you can put that in the witness statement.

Looks fine for this stage, as long as you had the claim details in the heading and signed/dated it, before emailing it to the CCBC.
See you at witness statement stage, early summer. 
PLEASE don't ask about any letter about your DQ, as this is fully covered in the NEWBIES thread.

Yes, I have the claim details in the heading and it was dated and signed too.

MCOL website shows defence was received. From going through  a post about what happens after defence, I understand now I wait for DQ and fill that as recommended in a sticky thread.
