Renewing travel insurance when a routine check up is due

One of the reasons why I hate dealing with travel insurance is that I never know whether it is necessary to wait until a routine check up/scan is complete (assuming that I have no new symptoms/reason to expect a problem). Due to using HRT now, I am waiting for a precautionary scan but this could apply to any routine check up. If we hold off booking trips every time, we could have a long wait. Sometimes it seems that a sensible patient is punished for being cautious. It can cause problems if someone wants to book a new trip after their current annual insurance policy expires. I would never do this because I might find that I can't renew straight away. Based on this, I have the following question:
If a woman decided not to have a routine mammogram or a man decided not to have a health check (prostate perhaps) and went ahead and booked a trip, might an insurance payout for a cancer diagnosis in the future be refused if medical records showed that he/she had opted out of a check/scan?
If this doesn't happen, the system seems to reward those who don't take precautions and so don't know that they have a condition.
