R40 Form

I am trying to fill the R40 form out for PPI tax refund. On the 1st page it asks for Your reference and Tax reference. Can anyone tell me where I would find these? Checked on a few letters that have come from HMRC but can’t find anything.

Just put your National Insurance Number as 'Your Reference', don't worry about tax reference0

We've just done one for my husband and one of the guides I saw said to put your NI number in there, as BoGoF suggested.
We also added his payroll ref too, as he was employed for the year in question and it appears on the P60 that we took salary numbers from, although I'm sure that's superfluous.0
