Can you get ex to give you letter (or email today)? Do you have a copy of the CB forms?
If not you can always supply this information later. As long as you provide it within your first assessment period (within one month from date of claim) you will get the child element included in your entitlement.
Note that if you have been off work caring for your child but your partner was getting the CB you have not been getting NI credits during this time.
