Replacing concrete patch

I have a patch of concrete/cement in front of my garage which is starting to break up and am thinking about getting it replaced. It covers s small area(2mx3m). What would a ballpark figure for cost be?

Why is it breaking up ?
If it is due to poor sub base, the whole lot will ideally need to be dug out, hardcore put down, and then concrete. The broken concrete could probably recycled as the hardcore, and then you'd need a bag & a half or two of ballast - £150(ish) on materials, a day or two of labour, and possibly a small skip.
Paying someone to do the entire job, maybe £500-£600 - More if you're in London.Her courage will change the world.
