Egg card PPI claim upheld - no enclosed acceptance form

I've just had a PPI complaint upheld from an egg credit card from years ago. Great news but they've asked me to sign and return an acceptance form before they'll send me a cheque. Problem is the form hasn't been enclosed! Has anyone else experienced this? there were no contact details on the letter (surprise!) but it's from canada square ops. Thanks!

You should really be able to accept the redress without the actual form, presumably there is a complaint reference number on your offer letter?
Write to Canada Square either accepting the redress by formally signed and dated letter or requesting that they send you an acceptance form. Or do both..
An annoying delay, but nothing to get too het up about...0 -
thanks - i called the FOS and they gave me a number for Canada Square who told me the form isn't needed.0
