Fidelity to iWeb ISA transfer

Hi,
I currently have a stocks & shares ISA with Fidelity (through Cavendish Online) holding the following funds:
Fundsmith Equity I Acc (GB00B41YBW71)
HSBC American Index C Acc (GB00B80QG615)
Vanguard FTSE Dev Wld ex UK Eq Idx Acc (GB00B59G4Q73)
I'm wanting to transfer these over to iWeb for 2020/21 tax year.
I've checked that all 3 funds exist on iWeb.
This is the first time I'll have done a stocks & shares ISA transfer having only done cash ISA transfers in the past.
Looking at the iWeb transfer form - https://www.iweb-sharedealing.co.uk/PDFs/ISA90DayShareTransfer.pdf, I'm confused
as to what questions 2 and 4 refer to.
Would I need to complete 3 of these forms or just one, as in one form for each fund I hold?
What would I need to write as the Company Name and description of shares in question 2?
Does question 4 apply at all (I don't hold any physical share certificates)?

On iweb’s forms page under “share dealing isa forms” the headings in italics are in the wrong place, appearing after the relevant sections. So this form is the wrong one, as it’s for “Transferring investments from an HMRC approved Save As You Earn (SAYE) option scheme or a Share Incentive Plan (SIP) into a Stocks and Shares ISA.”
You need to follow the steps above the link for this form, ie open an account and complete the online form.0
