How did he manage to get the instore credit cards or phones in person?

I am sure you need ID, statements etc to get goods in person. Usually all this stuff is delivered to your house

Anyway, back to the subject matter, I have now found out he took out phone insurance for 3 of the 5 phones he took out in my name. Can anyone explain why he would do that?

But the insurance plans were in my name and address. Surely the replacement phones would need to be delivered to my address?

Was the insurance taken out in-store when the phones were purchased?

If so, that might have been a bit of 'social engineering' by the fraudster.

The store salesperson would earn a big chunk of commission for selling the insurance. So the salesperson might be more likely to 'turn a blind eye' to some suspicious circumstances, in order to earn their commission.
