I did put a hole through the ceiling of our living room. The hole was far smaller than that repair. It was fixed, by a plasterer, so well that there's nothing to see.

Your repair is far too large for just a foot hole. Maybe an old loft hatch, a water leak, or the ceiling had to be cut back to the joists in order to fix it. Who knows until you go and have a look?

I have a two bed semi. There is a loft hatch in the corridor between the bedrooms so I don’t think it would have been one in one of the bedrooms.

I can have a look but I really need someone to tell me what I’m looking at. The roof also doesn’t have boards. My ladder also doesn’t fit. So I’ll get a ladder and see what I can see :) thanks everyone. In 40 years I will gain a lot of knowledge and be the one giving advice haha.

Remember to take your phone with you and take some photos of the area above the patch in the ceiling. If there's insulation covering the area, pull it back so you can see the ceiling below.

Be careful what you're doing, though - don't hurt yourself!
