Caxton - DCC dispute

In early August I was travelling in Germany with a Caxton prepaid multicurrency Mastercard with a Euro balance.
I used an ATM to withdraw EUR 200.
The ATM repeatedly kept asking me whether I wanted to be billed in my home currency.
I replied "no", but was still billed in GBP, which is not what I intended.
I believe that DCC should no longer be offered for prepaid multicurrency Mastercards, since April this year.
My withdrawal of EUR 200 cost me GBP 205, which was then converted by Caxton to EUR 228 because there was no sterling balance on the card.
I raised a dispute with Caxton on 8th August.
When I chased them I was told that it would be sorted out within 56 days.
This week the ATM transaction has appeared on my statement again, this time a debit of EUR 200, but the EUR 228 debited earlier has not been credited back.
I'm running out of patience, how do I escalate this?
