When working on a self employed basis and you are reporting a change of circumstances for tax credit purposes, do you put zero for 'Estimated anual employed earnings' and the amount you earn for 'Estimated anual self-employed profit'? What happens if it says 'This info is for April 2019 to April 2020 and you only started the job in September 2019?

Yes zero, if you are not aware of any income from your self employed business.
The comments I post are personal opinion. Always refer to official information sources before relying on internet forums. If you have a problem with any organisation, enter into their official complaints process at the earliest opportunity, as sometimes complaints have to be started within a certain time frame.

The information is for the whole year so my understanding is that if you have already had some employed earnings since April 2019 you need to include these.
Information I post is for England unless otherwise stated. Some rules may be different in other parts of UK.

Do I calculate my earnings from when I started the job (September 2019 to April 2020) or do i work out what I would earn over 12 months? Plus include my earnings from my last job from April 2019?

You need to report your employment income from April until you ceased/

You also need to estimate your self employed income from September to April.
