popla appeal and Sainsbury's ECP

had a medical emergency and was delayed in getting back to my car, ecp rejected my appeal.
They sent reply by email.
I'm now about to send a popla appeal.
Can someone point to a recent template to send, with generic points about signage etc,
It's all a bit confusing.
I've approached Sainsbury's by Twitter.
Will send head office a letter tommorow.

Complain direct by email to the CEO of Sainsbury's asap
Leave the popla appeal to about day 30 following the issue date
