I've been looking into leasing and these two companies seem to have decent deals and the additional options you mention:
Nationwide vehicle contracts - option to include both maintenance and insurance on some of their lease cars they call it their "complete care" package.
Motorama - offer the maintenance option but also £500 damage cover (so you'd still need to arrange your own insurance cover)0 -
