Land registry - which name on form TL1

My ex husband is transferring the title of a piece of land to me which was unfortunately missed in the main transfer of the house when I bought him out (there were two titles and one was missed).
He has since changed his name from the one listed on the title deeds.
When filling in the form TL1, which name goes in the "name of transferor" box? His old name or his current one? Further, when going to a conveyancer to have his identify verified, he won't have any documents in his old name.... What's the solution?
Any help appreciated as this has only been noticed by anyone now I'm halfway through the sale process:(

I would write 'AB, formerly known as CD'.
Does he not have an old passport?

How has he changed his name? Deed poll etc?
He would need to execute the deed under his current name and you would need to provide the Land Registry with the legal instrument which gave effect to the name change.
When someone changes their name by marriage for example, and they haven’t updated the Land Registry, they sign in their new name and then provide their marriage certificate (or a certified copy if a conveyancer is making the application).0
