Initial PIP Phone Call

I'm just trying to make sure my cousin has all the information to hand for when she rings up tomorrow to start the PIP process.
She has two eye conditions so therefore see's two opthalmologists, as well as a neurologist for her migraines and a consultant and a diabetes specialist nurse for her diabetes, does she have to give information on all these people she see's? As well as her GP ? WIll they ask this on the phone or on the form?
