Willing to learn, I said earlier we just had a piece of paper in each letter, but when I showed this thread to my partner he said we did have a form and got it - he's started to fill it out now.
They don't look like the ones in the video, but they do have all the things on them the text list says, all 4 pieces, except the form number is bottom left not right and rh whole thing is white with a blurry crown logo at the top.

No offence but no one is asking (or, to be honest, cares) about your ADHD or anything else.
You came on here asking for advice regarding a CCJ not medical advice.

Contact either the DCA or the issuing court of the CCJ on Monday morning and figure out what is going on and how to now get the debt settled.

I was answering your question when I said that. Nothing more. I don't expect people in different circumstances to understand that part.
