Applying for another provisional license after I got married and changed my name

I had a provisional license years ago in my maiden name. I never bothered to take driving lessons so I ended up just throwing it away.
I married my husband about five years ago and with every other organisation my surname has been updated.
When applying for my another and new provisional license, do I need to mention that I had one previously in my maiden name?
If someone had a provisional license years ago and then changed their forename and/or surname, does he or she need to notify the DVLA when applying for a new provisional license? I know if someone currently has a driving license in their former name and needs it to be updated then he or she needs to contact the DVLA to get their details updated. However, what is the procedure for someone who has a different name now compared to what it was before when he or she had a driving license?

Your old licence was valid for 10 years, so you could get a replacement. But that costs £20 and you’d have the faff of changing name and address. Easier to start afresh.0
