Universal credit uc 50 plus upcoming appt with jobcentre plus ..

Howdy !

I received the above questionnaire to fill out yesterday . First time ,and obviously want to make sure I do as required .
I have a couple of questions , if somebody please could advise ;)

I have a lot of documentation that is copied and ready to go.
All well and good , but tbh the enclosed envelope is too small to return all the necessary information .Could I stick the envelope onto a larger one to send back?

Also , I have my next interview at the jobcentre plus in sept for
" further evidence " . I was under the impression that what that meeting was about , was to produce my medical history to them at that time . I've only had the one meeting with my job coach back in March this year when I first joined universal credit . I did / do expect assessment , but after filling out the form I'm not sure what " further evidence " will be needed then .

I notice that UC do not want fit notes or medical statements . Fair enough , point taken . However , I can send medical reports from my doctor . Which leads me to my final question : Yesterday I received my 'copy of the letter sent from hsp regarding my care.'
This is from the specialist at st Thomas's hsp regarding my six monthly out patients visit last month . Am I allowed to send this ?

Oops ! One other question : Do I really have to put my national insurance no on all of the pages that I am sending ??

    I can't see any problem with that. Make sure you put enough postage on it and I suggest getting proof of posting. Also recommend that you keep a copy of the completed UC50.

    The DWP should let you know if they need more information. This might be related to any aspect of your circumstances affecting your claim. Keep an eye on your online journal and make sure you complete any tasks you are asked to do.

    For the avoidance of doubt, although they don't want Fit Notes, appointment letters etc to go with the UC50 you must keep supplying the DWP with Fit Notes to confirm that you are ill. Only after your assessment has been completed and a decision made will you be able to stop providing Fit Notes.
    You can send anything you consider relevant which speaks to your health and how you are affected by it.

    If you don't and the pages get separated from the UC50 the DWP will not be able to link them up - so it's in your interest to do this.

    yes... I almost laughed when I saw the freepost envelope... seems to me deliberately designed to deter you sending back evidence!

    But I figure paying for signed for delivery was worth it anyway to prove sent something back.

    Agree with above advice. Focus on the descriptors within the activities when describing your problems in each section and try to supply evidence relevant.

    Totally agree . A lot of off putting stuff . I doubt if I'll get anything inspite of doing everything that's required .I have a mild form of myasthenia gravis which makes me look 'normal ' apart from the excessive weight gain . However , the medical evidence I have explains the bad and the ugly . The admissions to hsp etc
    Not used to this ..
