Claiming PPI for deceased parent

After reading Martin's newsletter this week, I have realised that I may be able to make a claim for PPI for my late father. After reading Martin's template letter, it is not clear whether I need to put my details or my late father's? All of my father's accounts are obviously now closed.

How long ago did your father pass?
If it was some time ago, expect the Bank to no longer have any record of your father's finance.
