Transfer pension from scottish widows to II sipp

I've setup an interactive investor sipp, and want to transfer my old workplace pension from previous employment which is currently held in scottish widows. Value is £47k.

I've completed the II transfer form, but I need to fill in discharge form.
On scottish widows site there is a document library, no mention of discharge forms. I've seen some transfer forms but these maybe for transferring in.

Please can anyone be so kind to point me towards the correct form?

Also do I need to have taken advice from somewhere prior to transferring? During my googling of trying to find said form someone talked about a £30k limit.

The £30k limit is for final salary/defined benefit pensions or those with some sort of benefits/guarantees. If it is a straight DC pot then no limit applies.

Questions your existing provider may ask if you need to contact them for a discharge form

II have told you that you need to get a discharge form from SW?

Sorry, didn't get any notifications of a reply.

Yes ,ii have emailed a checklist, as well as the transfer form supplied by ii I also need to send them a discharge form. Does say if I have problems getting the form to contact them and they may help.

Ive emailed SW and not heard back from them. Might have to do it the old fashioned way and right them.
