Renewing passport but lost deed poll

Hi all, sorry for the lengthy post

I recently had a passport interview and they've sent me back because I need a change of name deed to show I've removed my middle name. On the gov uk website it says:

Titles and small changes to forenames
You can:

change the spelling of your name slightly - for example, Jane to Jayne
change the order of your forenames
remove middle names
Send 2 documents that show you’re using your new name. These can include a:

letter from a local council or government department
driving licence
bank statement
baptism or confirmation certificate

it doesn't explicitly state I had to bring a deed poll. I had two other documents with me that day

Anyway, I did remove my middle name officially 8 years ago but I have since lost the deed poll. I previously ordered it from one of those deed poll websites. My question is if I do get a new one, do I state what year I started using the name? I don't know whether I will have to fill in a new application form because the date on the new deed poll will be after the date I signed the passport application.

Any help please, I don't want it refused again

I removed mine just by doing it. The passport office have never queried it.

Losing the deed poll is pretty common. The only way to insure against it is to have it enrolled - that way you can always get a certified copy.
