PIP diary example

I'm currently writing a weeks diary to send off as evidence before my PIP tribunal, but I'm pretty stuck at what exactly to write.
I've printed off a template I found online from Citizens Advice, but there isn't much help with what I can write in the explanations. I was just wondering whether anybody knows what I can write, or if there is an example of a written diary (for mental health) that I can read to know how to write it myself.

Could you not just write what happens each day and how it affects your life, and what care/mobility needs arise from that?0

That's what I'm having issues with, I'm not sure what I should write about which is where having an example would be a great help.
Also, there are 2 addresses on my appeal date letter, one being in Birmingham and the other in Chelmsford, which is where the appeal will be heard. Which one would I need to send my evidence to.0 -

Have you written a statement and sent this to the Tribunal Service?

write down what you do each day and how your mental health affects how you do it compared to someone who didn't have mental health issues.
I dont suffer greatly with mental health issues but physical disabilities...so if i wrote that i got dressed in the morning i would put that i need assistance to get dressed as to reach over my head with a top causes severe pain, due to numbness in my fingers i cannot fasten buttons or clasps so would mostly wear elasticated skirts/trousers or i would need someone else to do them up for me as i am unable to myself. I need someone to fasten a bra for me as a, i cant reach around and b, i cant fasten it.
i struggle to bend down to put on any socks or shoes so need help with this too.
I would also add that getting dressed takes me about half an hour and by the time i'm done i am physically exhausted and need to rest for another 15-20 minutes before moving anywhere...
This is in total contrast to how i used to get dressed before my disability affected me this way.

You should look at the PIP descriptors to see where you think you should have scored those points.
For each descriptor that applies to you, you should write why you think you should have scored those points and your reasons why. Adding a couple of real life examples of what happened the last time you attempted that activity for each descriptor that applies to you.0 -
You need to ensure you really spell out how things affect affect you and how you would be negatively affected if you don’t get the care you need.
My son was awarded zero points on the back of a very inaccurate f2f report- I pointed out every single discrepancy and how his health, education and daily life are affected if he doesn’t get the care he needs. The judge awarded 20 points so he now receives standard for care and mobility .0
