Help! I’ve got two separate credit reports in my name?!

Hey I’m wondering if anyone can help me.
Basically, I purchased a new build house in May 2018 and took out my first mortgage. I registered to vote at my new address however I didn’t update my address for everything immediately and it was quite a slow process - however I noticed that as I updated my address for each credit account that it would disappear from my credit report as if the account had been closed?
My mortgage was never added to my credit report and neither was my new car finance that I taken out in my new address.
Today, I decided to look into it and registered on clearscore with a new email address under my new address... there I found all of my credit accounts listed under my new address so it seems that credit reference agencies appear to have my as two different people?! Does anybody know why this could be and how I could go about fixing it so that all of my credit accounts go back to my old credit report account and my address is simply just updated?

Just ask each CRA to make sure they link the accounts.0
