Look at the links in the NEWBIES thread about 'know what happens when' and you will see that regulars have posted how to fill in the DQ and where to file & serve it.

That gets added to your costs schedule that you file & serve with your Witness Statement and evidence before the hearing. And you ask the Judge for your costs when you win v UKCPM.
