Edit: I did a search and there is a branch in my town. So don't give up on it too quickly - like I was going to.

Eidt again: It worked fine when I typed the info, instead of using a form filler.

Same here but when I turned adblocker off the options appeared
so I could then make the choice !

Manually typing my postcode worked for me as I got no options on autofill

Manually typing in my postcode in lower case and no spaces brought up a 'box' with address details and after clicking on the correct one then I could choose an optician, so maybe try that way.
