iPad is disabled

In my company we use lots of iPads at our restaurants.
Every now and again I get the 'iPad is disabled' message and I need to unlock them. However, since the past few weeks, the number of devices showing 'iPad is disabled' has increased considerably.
Now, I don't personally use iPads so I don't know how an iPad can get disabled, apart from entering the wrong PIN too many times.
My question is: before I start to blame restaurant staff doing it on purpose, is there any other reason why an iPad would lock itself in disabled mode?

A quick Google suggests the problem is caused by:
1. Entering an incorrect pass code
2. An iOS update
3. A hardware fault (e.g. on screen keyboard)
3 should be fairly easy to determine - check that they're using the default Apple keyboard.
For 2 then check when iOS was last updated.
If it is 1 then suggest the employees receive "additional training".

No idea, sorry. It's easy to find out what version is installed (Settings > General > About), and when that version was released (Google), but I don't know how to determine when that release was applied to a specific device.0

Last update was rolled on 7th of Feb. Though I’ve never had that message on any update I’ve done over 10 iOS devices in the last 3 years.

Is the screen timeout coming into play just as they start to enter information, and the Ipad is seeing this as erroneous pass code entries?0

Were the iPads purchased as part of a package of related IT systems?

It's a rather simple setup actually: they use their own Apple ID and password (which I have been given) on the iPads, remove everything and leave one app only.0 -
If you have another iOS device use find my phone and reset your iPad then set it up with a new passcode.
If that goes well then try iCloud or iTunes restore from backup.
I had shocking time once when both iPhone and iPad went to disabled one Sun morning.
Though Apple agreed to unlock over telephone only after I requested and sent original proof of purchase from o2 took a few days.
Don't let the intermittent disabled message continue.

I've never had that message in eight years of owning numerous iPads.
If in doubt, "the simplest solution is almost always the best"; someone is either accidentally or deliberately entering an incorrect passcode.Every generation blames the one before...
