Applying to move gas meter

We have a gas meter which needs moving. It is currently situated on a wall which is being removed. Transco visited yesterday and asked us to contact National Grid as it was not an emergency. Although it is situated on an unsafe extended part of the property, which must be removed due to asbestos on the make shift roof, and poor brickwork.
Still, the gas isn't leaking so I see their point.
Apparently, we can obtain the form in which to apply for the meter to be moved on the National Grid website. I have looked but cannot find!
Does anyone know where I can find this?

The reloaction of a gas meter is a supplier obligation. The supplier in due turn will sub contrcat teh work to National Grid but it is most definately them who instigate works.
For a relocation of just a few meters should cost approx £4000 -
Thanks for your reply. Time is more the issue than cost. I was hoping to get the ball rolling quicker than waiting for the form to be sent!
