Well according to anselld's link you can't have an internal manhole anymore, but there must be an awful lot of them around. Nearly every house on my street has an extension built above the parallel sewer.
Sadly I can't extend because the geniuses that installed it made it higher than my floor-level as it passes through my garden.0 -
This wasn't "back in the day" , it was fully covered by the building regs, DS checking every step of the way and signed off.

I know it used to be possible. I worked in a local authority building for many years where the sewers were accessible via the floors, but all that's now irrelevant so far as the OP and modern building regs are concerned.0 -
