Travel insurance pre-existing health condition

Hello,
I'm sure I'm one of many that have asked but am somewhat overwhelmed with results - my wife had a heart attack over a year ago and this summer we're travelling to the USA, I've read MSE's article on travel insurance and like all these things have 101 results from various searches...when reading reviews they all seem positive unless someone actually makes a medical claim - is it just pot luck how the insurance company assists should something happen abroad or are they any better insurers /brokers from people's experience?
I imagine it's like most insurance, it's great until you need to use it
- but with health I want to minimise the hassle if possible.
It's my first time looking for insurance since this health issue happened so trying to find my way.
Any thoughts appreciated.
