PPI Questionnaire - what now?

Morning all,
Last week I did a PPI Check with various banks and credit card companies that I've used over the years. I've always knowingly rejected any extras I've ever been offered, but despite this it looks like at least 3 companies have confirmed I've had PPI; Lloyds, Halifax and Natwest, with possibly more to come.
Out of the 3 that have confirmed it, only one has really provided any Detail... Natwest have provided a list of all loans they found against my name and address and most of them had no PPI, but one of them did with a premium of around £500.
All 3 have included a questionnaire to start the claim; but half of the questions I don't know the answers to - for example.. What is the PPI related to - credit card, loan, overdraft, insurance.... I've not got a clue? Do I simply write on the form "Unknown"? do I phone them first and ask?
I've not got the questionnaire's to hand so cant give more examples (i can post later if needs be) - but if anyone has any advice on how they dealt with the questionnaire that would be great to hear.
PS - I just wanted to say a general thanks to everyone on here that keeps banging on about not using these claim companies; i came on here last week searching for "cheapest ppi claim companies" thinking I dont mind losing 10-20% because i just dont want the hassle" - but everyone is right, starting the claim just means searching for "Lloyds PPI Check" (replace Lloyds with whoever your lender is or was and running through a very simple form - all you need is your name, DOB current address and any previous addresses you would have used! don't even need a stamp for most of them

You only need to answer what you can remember, but it would be worth asking them for any further details they can provide you (particularly as to type of account).
The important thing is not to make anything up.0 -
Single premium is good loan complaint reason as is good sick pay, savings or cover elsewhere.
As above, don't lie on the form, if you can't remember, say you can't remember.
And as above, if you need more info, send them a SAR. They will send what info they have.
No need to post the questionnaire, we've all seen it or one like itNon me fac calcitrare tuum culi0
