    There's absolutely no chance I could fit stairs at 42 degrees anywhere without extending the whole house

    What is occupying the void space above the existing stairs from ground to first floor?

    What do you mean by 'proper joist'?

    If they are 'proper joists', then getting a professional to confirm things could leave you with a relatively small job of insulation plus stairs with corridor access to clean air at ground level.
