Thanks to all of you that made useful suggestions. An update: the First Utility direct debit mystery was probably my own fault. I used my own bank details when switching on behalf of my mother because I did not have hers handy, and I forgot to change this. This is the simplest explanation - there are more complicated ones that allow me to blame others, but they are less likely.
I found an old email that I had missed, apparently from British Gas asking for a deposit. It looked just like a scam. I do not know if it is a scam, or if this is why my account never switched. I don't know. British Gas are proving incredibly unresponsive. Cheap Energy Club have been quite helpful, off and on, but I haven't heard why my account did not switch, and why I was told 'switch complete. I may never know. I'm going to switch to Octopus.0
