Thank you for the unprecedented response to our survey, as we have had such a huge response we will be closing the survey early at 1pm this afternoon.

Apologies to anyone who hasn’t had a chance to complete the survey yet, or those that experienced difficulties in completing the survey. We had exceptionally high demand yesterday evening.

If you click on the link in the OP it should take you to a page where there's the option to 'TAKE OUR SURVEY', and clicking on that option opens a page where there is a space to enter your email.
