Kite Eyewear - Opticians gone out of business?

Had an eye test at the KiteEyewear.com Opticians shop in Shoreditch, London. Ordered and paid for new glasses using a Visa debit card on Saturday 26th Jan.
I heard nothing further. Now they are not answering the phone or responding to emails. It seems that the premises closed the following week. Apparently there is a sign saying closed for refurbishment.
Any comments posted by unhappy customers on Instagram are promptly deleted. So I guess they've gone out business.
Recently I received an email inviting me to fill in a SurveyMonkey form to claim a refund. amongst other information that the form asks me to supply are:
This seems to be totally out of order for a genuine refund offer.
Is this some a scam?

Thanks. That's what I thought.
Although the email appeared to come from kiteeyewear.com in the first place, if it was genuine it's hard to see why the need all this information. Surely they could just refund the original card used. And who would use SurveyMonkey for this sort of transaction.
What are the prospects of a Visa chargeback? How do I go about it?

Same thing. Paid, 5 weeks later nothing. Recorded message on their phone.. Closed for maintenance, right.. �� Offered me refund via email, asking for account number and sort code. I just called Amex, they will sort it for me. Shame used to be a good shop, nice glasses.0
