Including interest in a Third Party Debt Order

I have a CCJ in my favor and would like to apply for a third party debt order. I started filling the form N349, but I'm not sure where to include the interest and the court costs. There are two court costs: 60 for starting the court process and 110 for the TPDO application, some amount X of the judgment and Y for the interest.

From par 6 of practice direction to CPR 70 I know that I need to include the calculation of the interest the dates, etc. but I wonder what to put in the fields:

"The judgment or order required the judgment debtor to pay £ A (including any
costs and interest). The amount now due is £ B [which includes further
interest].
"
In the first sentence they mention (including any costs and interest, so I assume its X + 60 + the interest grown between court application and the judgment (that's what is there on the judgement, but there's also the amount without 60).

But for the second sentence, I surely don't know. Should it be X+Y? X+ Y+ 60? X+Y+60 + 110?
Or maybe only X, because from what I read in the hmrc internal manual dmbm667140 (sorry, I can't post links) "you should complete this to show the balance of the judgment debt remaining unpaid, you should not include any interest that has accrued since judgment."
which is weird, as the form explicitly says "which includes further interest".

Any ideas?
