    Exactly that. I have done that in the past.

    If you phone diabetes UK they will give advice on things to help you with your diabetes management. The advisers are very very good and helpful.

    Question regarding the form, I think its great I can type my answers up and attach as this will be easier for me, so is it acceptable to just put in the boxes "please see attached sheet number x" for example?

    Also write name, NI no., on each attached sheet.

  • When you send the form try and include the assessments the sight service carried out too and also any current ophthalmologists letters from the hospital.

    I'm not sure what other evidence apart from the diaries as you will need to explain how you struggle with certain tasks.

    I just wanted to come back and say thanks once again for all the information and help.

    With having Diabetes and also been under a neurologist for migraines too.

    Is it worth me holding off from sending for a application form until I have my next appointments with the Diabetes specialist and Neurologist so I have up to date letters to send with the application or just ring up for a form now and give the last letters I have with the form when I send it back?

    I'd be inclined to get the form now as the main issue for PIP is sight. The migraines and particularly the diabetes are more difficult to fit around the descriptors, so confirmatory letters for these conditions are less useful.

    You can also send in any letters you receive after returning the form as additional evidence with a simple covering letter.

    Write your NI number on any evidence you submit (and your name if not already present).
