She looked after the baby very well for about 4 months but didn't tell anyone she had a baby or registered the child.
So the baby wasn't registered with a gp, and therefore not being checked and weighed regularly, nor signed up for vaccinations? That is not "looking after the baby very well".
Removing a young child from a birth mother is an extreme step, and will not have been taken lightly.
