12 month wait for iva completion certificate

Hi all, we completed a 7 year IVA last January 2018 and are still waiting for our completion certificate!
They have sent us a letter confirming that there is nothing else outstanding and that there isn't sufficient equity in our home. They have claimed all the PPI we were entitled to as well. However, they seem incapable of collecting an endowment fund from Clerical Medical which we have given full authority for them to do. We have made countless phone calls, sent emails, complaint letters and were even promised paperwork the week before Christmas but surprise, surprise it didn't arrive!
It has been a year! My husband is in his 60's, so we are desperate to put plans in place for the future and are being left in limbo.
Does anyone have any advice on what we can do?
Our IP is Aperture, formerly known as Grant Thornton - absolutely hopeless! I would strongly recommend that anyone who needs to enter into an IVA does NOT do so with them.

How long ago was your complaint? I would refer to FOS if within the timescale as not resolved.0

I was with GT and the Apeture - got my certificate withing 6 weeks after completion, once we signed a waiver of PPI claims.
as above have suggested - formal written complaint with deadline in it, then to FOS0
