Not entitled to SSP - employer difficult to contact/reach

Apologies for my long post in advance.
My online staff account for my employment says I am to be paid sick pay but there was no indication of it on my wage slip.
I have used the SSP calculator on the gov.co.uk website to see if I am entitled to sick pay, but apparently I am not. I fall slightly below the threshold.
Now, knowing that this calculation could have been performed in the first 7 days of my sick leave by my boss, I am asking what I should or can do further?
I have read that I can possibly claim ESA by completing SSP1 online, printing it off and my employer signing it, but one question stumps me: 'Was your sickness caused by an accident at work or an industrial disease?' Googling this gives me compensation websites or solicitors/lawyers. I don't want to get this answer wrong, so is carpal tunnel an industrial disease??
I have tried to contact my employer since Friday by way of 2 phone calls and 2 emails and he has not responded and having rung work this morning, he is off today. I am also trying to retrieve my sick note from him. I took a photo of my sick note when I was given it. Would this be a suitable replacement if he has misplaced it?
I am due to contact the head office/payroll department and also the Jobcentre for advice this morning when their respective offices open.
Any advice or further steps to resolve this would be appreciated. Thank you

If you have no entitlement to SSP your employer is required to provide the SSP1 with the reason you have no entitlement. The SSP1 and medical certificate are required to support your claim for sickness benefit.
If you aren't earning enough to qualify for SSP are you sure you should be claiming ESA? The Contribution based version 'New style ESA' requires that you have sufficient NI contributions in the 2 qualifying years. If you don't qualify you will need to claim UC.0
