Changing name with Land Registry

Hi,
I've recently got married and I'm going through the process of changing my surname on everything and I read that it's important that you change your name on your house deeds. On the HM Land Registry they say that to do this you have to complete an AP1 form and send it along with your marriage certificate. The problem is I am really struggling with filling in the form! I don't understand any of where I put my old name, where I put my new name - I've read their guidance on filling in the form but I'm still very confused. For something that must be a common occurrence, I feel like I must be missing something?
How did everyone else go about changing their name on their house deeds? Or have most people not bothered?

I've not changed mine on house deeds. I still haven't on driving licence and 1 bank account. I just go by both names.Forty and fabulous, well that's what my cards say....0

Relatively simple and lots of help out there as one of sisters did this.
Go to the .gov site/land registry and get a pdf form TR1
and ID1

Summarise
TRI form
iD1 for your id and anyone else on the deeds and you will need it certified.
Deeds to your property will include title number and other information required.
And Land registry staff are very helpful bar the odd one and get calls like you daily
Once completed send with the relevant fee/cheque to your local office
NB, the youtube vids, pause, fill in form, go back etc and it gives a good example of a situation like yours

You don't need to change your name - my wife hasn't - but she's known by both maiden and married names.

When I got married, I forgot all about changing my name on the house deeds.
When we came to sell, it was flagged as an issue that was resolved by a "certified copy" of my marriage certificate.0
