I use Microsoft RegClean. No longer available from MS, but plenty of sites have it.

tweaknow is free but doesn't remove as much as others.

Ace Utilities is fantastic but ony a free trial - Very highly recommended
(the free trial works straight away - no sending away for registration etc)

I've just used Reg Mechanic and it found over 1000 dead and useless entries. Free for 30 days, not restricted and only about £12 to buy.

I thought the registry mechanic trial was very restricted??? -
when I tried it they only scanned a small part of the registry - you had to pay for the full version to do the rest.

Why should you clean a registry? What happens if you delete an important file?

the tweaknow link above takes you to a page that tells you about registry cleaning.

I've never seen a registry cleaner that doesn't backup the registry first (Unless you disable the option)
