My adult daughter was assessed for PIP from DLA 2 years ago .
She was awarded Enhanced for both categories from High care daily living and Low mobility .
Her physical health declined over the years but with complex mental health difficulties she was unable to face being assessed while the DLA was always renewed on paper .
With PIP we updated her difficulties with lots of reports etc and she was fortunate to have an experienced nurse assess her at home
Today an ESA 1043 form has arrived nearly a year before her 3 year award expires . I was aware people had begun receiving these forms quite early but am quite confused by the limited information requested on the form .
I have searched the board prior to posting this and found one thread mentioning this form . Alice has kindly put links to CAB and B&W on that thread in Feb this year .
B&W are referring to AR1 . Is this the same form or a different one to the 1043?
I am concerned at the lack of information asked for this time . I had already decided to enclose a supporting letter from her GP although they do not want one . She is lucky that she has only had two GPs at the same practice in 33 years and the current one has been caring for her for the past 16 years with 2 weekly appointments all the year round . He knows her as much as I do .
I'm hoping even if his letter is ignored if things go wrong it may help with any further action needed .
As always there will be lots of pages clarifying day to day problems included with the form .
I would appreciate clarity on whether both this form and the AR1 are the same .
Any further links or advice would be helpful . We've had an exhausting year of many appointments and I need to get my brain working on this .

I'm sure they changed the form last year so it's probably the same thing.
Definitely write as much as you need, don't rely on "no change" and use continuation sheets if necessary. Write your daughters name and NI number on each page.
Make sure you send away any relevant evidence as there's a good chance that whoever reads her case won't see her old file. A letter from the GP would be ideal.0 -

Thank you for your reply . My instinct was to send them an updated version of the extra info I sent with the PIP assessment . I too was thinking of problems re the old file . When she was sent the original PIP form they asked on the phone if we wanted the DLA file looked at . They then told me they couldn't find it so it was lucky they got previous as well as present history with the form .

Yes, the PIP review form has changed. I've just sent my daughters back for her first review.
My advice, treat it like it's a first claim and put as much information as possible about how her conditions affect her. If you need more space to add the information just use separate A4 paper, this is what i did. I typed out the extra information, making sure i put which descriptor it was related to (with the number as well)
Don't forget the 14 days extra time, if needed. I rang and asked for that and they happily agreed. Good luck.0 -

A letter from the GP would be ideal.
A letter from a GP was on the list of what they don't want to see. Most likely because a GP doesn't really know how your conditions really affect you.0 -
Thanks poppy and to Jen for both confirming the form is different . You've both confirmed my thinking on lots of info too .
After so many years of these forms I work on too much is better than too little .
Thanks for the extension reminder poppy . This is only allowing 3 weeks to be back with them . The original PIP claim did require an extension due to all the info needed and dds anxiety and panic attacks . We've discussed this one today and plan to tackle it next week . The good thing is it's not as thick as the PIP form and I have all her past paperwork filed including the PIP form and the paperwork from that .
She sees her GP next week so he will do a letter and a couple of copies . I plan to time returning it to be back in time but not early . Her birthday is just a week and a half away and Christmas is coming so although it's not likely she'll be called for a face to face so quickly I'm playing safe .
I always send next day signed for so will send a couple of days before it's due .

That was one of the things I was bothered about but have decided to send one anyway as he knows her inside out and monitors her all year round .
If they ignore it ok but if it has to go further( hopefully not ). I will have a dated letter from him showing her mental and physical condition at the moment as he always does a few copies .

Remember the Christmas post also when you return it and using next day delivery when sending to DWP isn't next day delivery. Post is sent to a mail sorting office and will be signed for by someone from Royal Mail, not DWP. It can still take several days to arrive at the correct department after it's signed for.0
