Change to tax credits - Universal Credit?

Hope someone can help me. I currently claim working tax credits and am employed on a low income. As of next year, I will need to change my working tax credits to self-employed (potentially on a lower income) I'm also disabled.
Will this change mean I automatically switch to Universal Credits? I am in a full service area. There are no guidelines as to what happens when you switch from employed to self employed.
Thanks in advance, this whole thing is a nightmare.
