Did you take copies? If so, could you take copies of those and send them urgently as a sort of safety net, whilst you are trying to resolve it by phone? Also, your MP should be able to do something. Many years ago a government department insisted that they hadn't received something from me. A phone call from my MP one morning and the papers were miraculously found that afternoon. MPs usually do urgent appointments where you would probably see one of his/her assistants.
I hope it gets resolved quickly.:snow_grin"Let it snow, let it snow, let it snow........":snow_grin0 -
Have a look at this, the problem has happened before:

While I try to check everything is correct before posting, I can and do make mistakes, so always try to check official information sources before relying on my posts.0 -
I would suggest (not in any particular order):
a) Requesting an Mandatory Reconsideration of the decision to close your claim (include a copy of the proof of delivery).
b) Try ringing again and see if you can speak to someone more sensible / senior.
c) Contacting your MP and ask them to intervene.
d) Making a complaint.
e) Asking them to proceed straight to assessment.
f) If your disabilities are linked to depression / mental health, the DWP should be more flexible around return dates - if this applies go to your local CAB and see if they can ring on your behalf (or get a friend / carer to ring the DWP)
If you are now in a UC area - be wary of claiming UC instead of ESA. Please seek advice from your local advice agency / CAB first.
I'd be interested to see what other forumites suggest.
Hope the DWP see sense and you get it sorted.Alice Holt Forest situated some 4 miles south of Farnham forms the most northerly gateway to the South Downs National Park.0 -
If the letter containing the form was signed for how can they claim it was never received? It should be DWP's problem now to actually find it. Beyond that, follow the advice from Alice Holt.0

Mine advice is ALWAYS photocopy when you send to DWP. Send it by special delivery. Photocopy the delivery slip too.
Initially when claimed ESA, they couldn't find copy of passport! Sent it again. Then they couldn't find my fit note! Both were sent by SD. I just photocopied the sick note again and sent that with previous delivery note.0
