Including past PIP assessment as evidence

I was thinking of putting in my assessment for my initial PIP award in with my review forms.

On the whole it's good, I was awarded higher rates on both.

It's quite long, do I include the whole thing?

Also, there are a few things I disagree with on there, such as I have black outs, I need someone with me to ensure I am safe because of this as they have happened in a number of public places. The DM said I had warnings of them, so that doesn't matter, I disagree with this.
Do I still include this, even though I disagree?

It does state at the end I am unlikely to improve and will probably be the same on review, so that's favourable and a number of things are.

Any help on this would be appreciated. Thank you.

    Personally I would say 'no' as they are interested in how you are at the moment and not how you are when you were first assessed (how long ago was this?)

    What I would say is to treat this review form as if it is an application and include everything you did previously plus any changes.

    It is also vital that you get some up to date evidence from any health professionals particularly as it is a review and they need to see that things have not improved (if that is the case).

    It was two years ago, and I'm pretty much the same as I was with the same issues.

    I've been told to include the medical by others, so unsure what to do.

    I'd suggest including on the new form an additional comment along the lines of "At my last assessment (xx/xx/2016), I was awarded enhanced rates for both Care & Mobility. Since when my condition and abilities have not improved, I still experience the difficulties outlined in that report on the majority of days".
    But, I wouldn't send in the report.

    I'd be inclined to treat the review form as a full form, and spell it all out in full detail. Attach extra sheets, if you can type them so much the better.
    Enclose confirming letters from family / carers / etc if you really can't get up to date medical evidence. Can your GP confirm that you still need the same help with care and mobility activities as you did on xx/xx/2016?

    I agree with pmlindyloo and say no don't include it. They won't be interested how you were the last time, only what you're like now.



    Who advised you to include it?

    Yes, I will write it all out, it's going to be typed this time, much easier to edit too.

    Someone in my other thread said it was right to include it, and before that I had seen it advised elsewhere to copy the medical from the previous award and add it in to evidence.
  • Agree with what others have already said re how to approach completing the form.


    With regards to the previous assessment however, there is no need to send it because they already have it, and is easily accessible on your account to the decision maker, should they wish to view it.
  • OP do you warnings of black outs? if so for PIP they are not an issue, because if you do get warnings then you can ensure you are safe when they happen.

    Not every time no. Sometimes it's very out of the blue.
    And if I am alone, I cannot sort myself out as my warning as it is, is that I feel sick and confused and disorientated, so I still need someone to help me as I'm unable to sort myself out.
